• More than one location requires a schedule, over 8 stories requires life safety features to be listed for the building
• Underlying GL coverage required limits of $1/2 million must be met
• Hired non-owned auto coverage must be included in underlying policy with a limit of $1 million
• 5 years of GL loss runs for all locations
• Description of all GL losses over $25,000, including injuries
